We will need to know:            

1. Does the resident have a g-tube?                                                       

2. Does family or therapy wish to be present for evaluation?         

3. Is this a repeat study?  if so, approx date?                                      

4. Do you wish to have a digital copy of the MBSS?                          

5. Who is the scheduling Key Contact? 

Third – it is our policy that we will call you 30 minutes prior to our arriving at your facility.

The patients that are scheduled to be seen that day, MUST BE IN A WHEELCHAIR, APPROPRIATELY DRESSED AND PREPARED FOR THEIR CONSULTATION AND MBSS.

This is a valuable service that can only be provided to multiple facilities if we stay on schedule. We need your assistance to make sure all patients are ready to be seen. If not, we will have to continue on to our next location and reschedule your patients for another day.
 
Your appointment will be assigned a scheduled day. Based upon our route for the day, a time slot will be scheduled. The Key Contact listed above will be notified of the specific appointment time ASAP, but no later than Noon two days before the scheduled appointment date.

IF YOU HAVE ANY SPECIFIC QUESTIONS ABOUT WHETHER OR NOT A STUDY IS INDICATED, WE WILL BE HAPPY TO DISCUSS IT WITH YOU PRIOR TO THE STUDY.
