Mobile Dysphagia Consultations with the Modified Barium Swallow Study

Dysphagia Mobile Imaging, Inc. (DMI) is a professionally-owned mobile speech - language pathology clinic that provides MBSS to nursing homes, assisted-living and private home care clients who need timely and convenient assessment of swallowing safety onsite as opposed to offsite in the traditional hospital setting.

 

The era of transporting nursing home and home-bound patients from their comfortable environment to a large, obtrusive, hospital facility is at its end; as a result, the contract facility’s patients and staff no longer waste long periods of time in hospital waiting rooms where they are also exposed to other patients with various illnesses.

 

While patients who are transported to the hospital setting for the MBSS procedure are evaluated by a professional Speech-Language Pathologist (SLP), this SLP most likely does not know the patient’s eccentricities or their normal eating patterns. It is unlikely that the patient’s personal therapist, who knows them best, can accompany the patient to the hospital as they have plenty of patients to attend to at the facility.

 

DMI eliminates these issues and more by bringing the MBSS to the nursing home where the patient feels more comfortable and the MBSS will more accurately emulate the patient’s daily feeding regime. With the opportunity for the patient’s personal therapist and attending physician to participate in the study, DMI can assure the procedure is conducted with the highest of confidence for a clear assessment of the physician’s order.

 

The DMI mobile MBSS clinic provides a private, controlled environment for the assessment of swallowing disorders and eliminates the patient’s disorientation and fatigue that arise from transporting the patient offsite. The mobile clinic allows DMI to reproduce the patient’s living environment providing overall clinical findings that are more accurate and better representative of the patient's daily swallow function. In most cases, the patient can even be assessed in their personal wheelchair, further contributing to a more realistic assessment.

 

The state-of-the-art, mobile imaging clinic is equipped with a c-arm videofluoroscopy system, digital video & audio recording capabilities, and is fully accessible accommodating most populations.

 

A highly trained DMI SLP personally conducts all swallow studies with an assistant who has specific skills for capturing dynamic, motion images. A board certified physician is onsite reviewing the MBSS procedure and writes a separate physician's report based on their findings. All stages of the swallow are assessed, as well as how the cervical spine affects dysphagia. Radiographic views in lateral and A-P planes are available and immediate results with feedback by the SLP are provided to the facility for patient care.
 
In addition to attending the procedure itself, the facility SLP receives a digital video recording of the swallowing assessment to include in the patient’s medical record and use for follow-up evaluation and/or education.

DMI vs Hospital

1. DMI uses the same CPT codes as used in the hospital;

2. DMI Physician is present for every study completed on the DMI Mobile Clinic;

3. DMI only employs ASHA certified Speech-Language Pathologist (must have a Master’s) and intensive DMI MBSS training;

4. DMI continuously gathers QA measures and reports statistics back to each Facility to assist in each entities' quality improvement initiatives;

5. DMI Mobile Clinic is specifically designed for the MBSS procedure; hospital equipment requires modifications to accommodate MBSS patients;

6. DMI Videofluoroscopy unit provides quality lateral and A-P views;

7. DMI provides SLP and MD Reports at the time of the consultation with a copy of the study on DVD for Facilities Records; and

8. DMI can schedule Facility within 24-48 hours.
